1. My child is guaranteed no playing time. This is the most competitive division of GEYL and winning games is the objective.

2. Costs for this division are higher than playing in the in-town league. The initial payment is due by September 1, 2010. No refunds will be issued after October 1, 2010.

3. I will respect my child's teammates, opponents, coaches, parents, officials, administrators, and myself. Any misconduct on my part will be met with appropriate disciplinary action which could include suspension and/or dismissal from the team/league.

4. Travel is required for this team to compete. I understand that travel to and from the games is my responsibility and the expenses resulting thereby are each player’s responsibility. Additionally, it is very probable that this team will enter weekend tournaments and I understand and agree that my travel and accommodation expenses will be my responsibility.

5. I understand that I may be requested to fund raise.

6. The selection for this team is for the 2011 season only. I understand that my child will have to try out and be selected for this team each successive year.

7. All concerns and complaints must first be addressed by the coach of my team. If a satisfactory response is not received, I will then contact the league office.

8. I understand that this team may utilize guest players for certain tournaments, including, but not limited to, post season play, e.g., state and national tournaments. These guest players may encroach upon my child’s playing time. I know that the objective of guest players is to enhance the overall competitiveness of this team.

I ACKNOWLEDGE, AGREE AND UNDERSTAND THAT TO ALLOW THIS CHILD TO PARTICIPATE, I HEREBY ASSUME ALL RISKS OF INJURY OR LOSS INCURRED OR SUFFERED WHILE IN AND/OR UPON THE PREMISES OF THE Youth sports Complex or other facilities used by the Greeley Evans Youth League. I release and agree not to sue GEYL, its agents, servants, associates, employees, coaches or anyone connected with GEYL for any claim, damage, cost or cause of action, which I have or may in the future have because of injuries or damages sustained or incurred while in and/or upon the premises of the above mentioned. I, the undersigned parent or guardian, hereby give consent for emergency medical care or action deemed necessary by anyone present in order to ensure the most immediate treatment possible, which includes but is not limited to taking the child to the nearest health care provider, until the parent and/or guardian obtains their own medical treatment. This treatment may be given under whatever conditions are necessary in order to preserve the life, limb, or well being of the player above. All such care shall be deemed appropriate by me in order to ensure the safety of all concerned.
